Special Notes: Birdbrain Disc Golf Club was formed in late 2014, early 2015 by Anthony Perez, Kyle Fitzpatrick, Will Russell, and a few other individuals. Their original home courses were Shady Oaks in Streamwood Illinois and Black Bear in Hoffman Estates Illinois.
